Outreach involves the activity of providing services to any population that might not otherwise have access to services. It includes reaching out to assist those in need of food, shelter, financial assistance or social services. Change Me Foundation will partner with community organizations to provide outreach services to those in need ensuring that no one is left behind.
Health & Wellness
Wellness is a big problem when it comes to those living below the poverty level. Data demonstrates that many families, seniors, and veterans are in poor health due to a lack of health care literacy and an understanding of the importance of seeing a health care provider on a regular basis. Many individuals lack the understanding of their medical conditions, how to access proper treatment and how healthy eating can improve health conditions. Whether access is about personal food choices or related to cost, our goal at Change Me Foundation is to teach healthy eating and offer healthy educational classes on a monthly.
Advocacy is defined as an act of speaking on behalf of or in support of another person, or to defend or promote a cause. We know when it comes to advocacy those affected by poverty find it hard to speak on behalf of themselves and their families. As our very mission states, Change Me Foundation plans to advocate for those who have no voice. We plan to speak on behalf of those in need as well as educate and teach individuals how to advocate successfully for themselves. Advocacy training includes how to handle events which may require you to use your voice to navigate through the system to ensure that your voice is heard.
A support group is an opportunity to spend time with people that are going through similar challenges. Support groups are comprised of non-professionals that are there to share their struggles and coping strategies with each other, which creates a sense of community. Change Me Foundation will offer a safe community where individuals can feel comfortable in sharing their story. We plan to offer support groups in underserved communities as the need arises. While there are outreach efforts available in identified communities, few support groups are taking place, and support groups play a vital role in helping those in recovery from mental health or substance use conditions.
Education goes beyond what takes places within the four walls of the classroom. A person gets the education from his/her experiences outside of school as well as from within. The goal of Change Me Foundation is to provide mental health awareness education to our community. Our vision is to educate in the areas of mental health, advocacy, substance abuse, health & wellness, healthy eating habits and home ownership.
Community resources are assets in a community that help meet certain needs for those around them and they have the potential to improve the quality of life in a community. Change Me Foundation is looking to help those in need find access to resources around health disparities, hunger, substance abuse , mental health, conflict, jobs, affordable housing, pregnancy, HIV/AIDS, state economic benefits, education, clothing and etc. As part of our community resource initiative, we will provide assistance to walk individuals through the process of finding and/or applying for needed services or benefits.
